Senior Social Worker – Adult Social Care (Continuing Healthcare)
Location: London
Contract: 5 months ongoing
Hours: Monday to Friday, 9:00am – 5:00pm
Pay Rate: £36.54 per hour (Umbrella)

Service Care Solutions are recruiting on behalf of one of our London based clients for an experienced Senior Social Worker / Specialist Practitioner to support Adult Social Care (ASC) with work related to the Continuing Healthcare (CHC) pathway.

This is a non managerial position, focused on providing specialist leadership, guidance, and quality assurance rather than direct line management of staff.

Role Overview: You will play a key role in leading and supporting ASC staff with the CHC pathway, ensuring practice is robust, compliant, and aligned with the CHC National Framework. The post requires strong professional judgement, partnership working, and confidence operating across organisational boundaries.

Key Responsibilities:

• Lead and support Adult Social Care staff with work relating to the Continuing Healthcare pathway
• Provide specialist advice on the CHC National Framework and related legislation
• Work collaboratively across health and social care organisations to support effective pathway delivery
• Maintain strong professional relationships with external providers
• Undertake quality assurance to ensure standards of practice and decision making are met
• Contribute to improving practice consistency and outcomes across the service

Requirements:

• Qualified Social Worker with relevant professional registration
• Extensive experience and proven competence working with Continuing Healthcare (CHC)
• Strong knowledge of the CHC National Framework
• Experience working across organisational boundaries (Health and Local Authority)
• Proven ability to manage professional relationships with providers
• Strong quality assurance skills
• No line management responsibilities required
