Job Purpose We are seeking an experienced and dedicated Adult Social Worker to join a dynamic team in Norfolk. You will work collaboratively with individuals with complex needs to develop support plans that maximise independence, promote choice, and help achieve personal outcomes.
You will deliver an integrated approach to assessment and care planning, working closely with key partners across health and social care.

Job Purpose We are seeking an experienced and dedicated Adult Social Worker to join a dynamic team in Norfolk. You will work collaboratively with individuals with complex needs to develop support plans that maximise independence, promote choice, and help achieve personal outcomes.
You will deliver an integrated approach to assessment and care planning, working closely with key partners across health and social care.
Key Responsibilities
- Manage a varied caseload, undertaking assessments under the Care Act and relevant legislation using a strengths-based approach
- Develop, coordinate, and review care and support plans to ensure outcomes are achieved
- Support individuals to assess and manage risks within their community
- Lead Safeguarding Adults investigations in line with multi-agency procedures
- Handle complex cases involving safeguarding concerns, capacity, and best interest decisions
- Prepare professional reports for court, case conferences, and other formal settings
- Promote safeguarding awareness and maintain up-to-date knowledge of current legislation and best practice
- Work collaboratively with service users, carers, and multidisciplinary teams
- Qualified Social Worker with relevant registration
- Strong experience within Adult Social Care
- Confident managing complex cases and safeguarding enquiries
- Excellent assessment, communication, and report-writing skills
- Ability to work independently and as part of a team
