About the Role:

This role requires a proactive individual with strong assessment skills, excellent communication, and experience working within health and social care environments.
Candidate Responsibilities:
- Act as a key point of contact between health services, social care teams, and adult clients to ensure seamless care coordination.
- Conduct thorough assessments to identify clients’ needs, risks, and eligibility for support.
- Develop personalized care plans tailored to individual client requirements.
- Facilitate effective communication among multi-disciplinary teams to promote client well-being.
- Support the delivery of services that enable independence and improve overall health outcomes.
- Maintain accurate records and ensure compliance with safeguarding, confidentiality, and data protection policies.
- Lead case reviews and contribute to team discussions on complex cases.
- Provide advice, guidance, and support to less experienced colleagues within the team.
- Proven experience working as a Qualified Social Worker (QSW) within adult health and social care.
- Strong knowledge of adult safeguarding, mental capacity, and care planning.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Relevant social work qualification and registration with Social Work England.
- Experience in health liaison or care coordination is desirable.
- Employment Type: Contract
- 35.50 LTD inclusive of holiday pay or 28.60 exclusive of holiday pay
