Job Title: Social Worker - 35 (Adults)

We are recruiting for a Social Worker - 35 (Adults) to join a dedicated team supporting adults during inpatient stays at Norfolk and Norwich University Hospital (NNUH). This is an exciting contract opportunity to work at the heart of a multi-agency service, helping to facilitate safe and timely discharges and ensuring people receive the right support at the right time.

Social Worker - 35 (Adults) will operate within a Home First ethos, prioritising independence and recovery through strong partnership working. You will also support with reviews after discharge, whether individuals return to their own homes or into provided care settings.

Candidate responsibilities

As a Social Worker - 35 (Adults), your duties will include:

• Undertaking person-centred assessments with patients and their families
• Developing and implementing care plans that promote independence and wellbeing
• Working collaboratively with multi-disciplinary teams to ensure smooth discharge planning
• Applying the Care Act 2014 in practice to support effective decision-making
• Completing Mental Capacity Assessments and acting in a person’s best interests
• Supporting Continuing Healthcare (CHC) reviews and ongoing care coordination
• Providing clear, timely communication across health and social care professionals
• Working flexibly as part of a small team, including travel requirements across relevant areas
