Social Worker - 35 (Adults) – Contract (6 months) | Willow Therapy Unit, Norwich Community Hospital
Are you a qualified and compassionate Social Worker - 35 (Adults) who thrives in multi-agency environments? We are seeking a dedicated Social Worker - 35 (Adults) to join our team supporting adults during inpatient stays at the Willow Therapy Unit. This is a contract role starting ASAP for an initial 6 months, with the service operating under a Home First ethos to promote independence, recovery, and timely discharge.
Candidate Responsibilities
As a Social Worker - 35 (Adults), you will work at the heart of a multi-disciplinary team to facilitate safe and timely discharges and ensure individuals receive the right support at the right time. Key duties include:
- Undertake person-centred assessments with patients and their families
- Develop, review, and implement care plans that promote independence and wellbeing
- Collaborate with health and social care professionals to support smooth discharge planning
- Complete Mental Capacity Assessments and apply best interests principles
- Support with Continuing Healthcare (CHC) reviews
- Work with individuals once discharged—reviewing needs in their own homes or within provided care settings
- Participate in driving between visits/placements as required for the service
