The Role:
Lancashire County Council is seeking an experienced and motivated Social Worker to join the Adults Services team within the Short Term Support (Prevention) Service. This is a fast-paced and rewarding role focused on supporting timely and safe hospital discharges across Central Lancashire.
You will play a key role in triaging referrals and planning discharges for residents, ensuring individuals receive the right support at the right time to promote independence and prevent readmission.

- Assess service users and develop appropriate discharge plans

- Facilitate safe, timely discharges from hospital settings
- Ensure compliance with relevant legislation and best practice
- Must be able to attend ward visits at Royal Preston Hospital and Chorley Hospital
- Office base at County Hall, Preston
- Candidates should be local or able to travel reliably within the area
- Qualified Social Worker with relevant registration
- Experience in Adults Social Care, ideally within hospital discharge or similar settings
- Strong assessment and care planning skills
- Ability to work in a fast-paced environment
- Excellent communication and partnership working skills
