Our client, Derby City South Community Support Team, is seeking a dedicated Social Worker / Care Manager - Grade K for a 3-month agency placement to join their busy and supportive team. If you possess recent experience working with Liquid Logic (LAS), completing safeguarding investigations, and delivering person-centred care, this could be the perfect role for you.

Candidate Responsibilities:

As a Social Worker / Care Manager - Grade K, your primary responsibilities will include:

• Conducting face-to-face visits within Derby city areas, with some office attendance required as part of the hybrid working model.
• Completing mental capacity assessments and making Best Interest decisions in accordance with current legislation.
• Leading safeguarding enquiries under s42 of the Care Act and practicing within the principles of Making Safeguarding Personal (MSP).
• Developing person-centred assessments and strength-based support plans.
• Facilitating reviews to ensure care plans meet individual needs and support wellbeing.
• Navigating Liquid Logic (LAS) autonomously to update case records and manage workflows efficiently.
• Working collaboratively within a fast-paced community support environment to provide high-quality care and safeguarding.

Candidate Requirements:

• Recent experience using Liquid Logic (LAS)
• Proven track record in completing mental capacity assessments and Best Interest work
• Experience conducting safeguarding enquiries under s42 of the Care Act
• Knowledge of Making Safeguarding Personal (MSP) principles
• Ability to perform face-to-face visits in Derby city areas
• Strong communication, organisational, and decision-making skills
• Ability to work autonomously in a busy community setting
