About the Role: Social Worker - Discharge To Assess (D2A)
As a Social Worker - Discharge To Assess (D2A), you will be responsible for conducting assessments and reviews following hospital discharges, ensuring citizens receive appropriate support across various pathways. Your role will involve working closely with hospital teams, clients, and multidisciplinary professionals to facilitate smooth and effective discharge processes.
Candidate Responsibilities:
- Conduct comprehensive assessments and reviews post-hospital discharge for citizens across three pathways
- Develop and support discharge and ongoing support plans tailored to individual needs
- Cover duties related to Community Adult Assessments (CAA) and Support Planning
- Provide Best Interests (BI) decisions and Mental Capacity Assessments (MCA) where required
- Participate in safeguarding duties to ensure client safety and well-being
- Attend Quality Assurance (QA) panels to discuss funding provisions and support ongoing client care
- Collaborate with healthcare professionals, families, and external agencies to ensure seamless discharge processes
- Maintain accurate and timely case documentation in line with organisational standards
Candidate Requirements:
- Proven experience as a Social Worker within hospital discharge or adult social care settings
- Strong knowledge of D2A pathways, safeguarding, MCA, and BI procedures
- Excellent communication and interpersonal skills
- Ability to work under pressure and manage a complex caseload
- Relevant social work qualification and current HCPC registration
