About the Role

As a Social Worker Level 3 - Mental Health, you will play a vital role in supporting adults with severe and enduring mental health conditions. Your responsibilities will include assessing, planning, and coordinating care directly in community and hospital settings. You’ll work closely with individuals, their families, and partner agencies to promote recovery, independence, and improved outcomes, all within a values-led service focusing on empathy, respect, and collaboration.

This Social Worker Level 3 - Mental Health role is a 3-month temporary contract with the potential for extension, paying a competitive hourly rate (please contact us for specific pay details). The position involves working 37 hours per week, with flexibility to work four days if preferred. The role offers hybrid working, with approximately half of the administrative tasks completed at the office and the rest split between remote work and community visits.

Candidate Responsibilities – Social Worker Level 3 - Mental Health

• Conduct comprehensive assessments of adults with mental health conditions
• Develop and implement individual care plans
• Coordinate multi-agency support and services
• Support patients in their recovery and promote independence
• Work flexibly across community and hospital settings
• Maintain accurate documentation and case notes
• Engage with families and support networks
• Uphold confidentiality, safeguarding, and professional standards
• Participate in supervision and ongoing training to enhance practice

Candidate Requirements – Social Worker Level 3 - Mental Health

• Qualified Social Worker with current SWE registration
• Proven experience working with mental health service users
