Location: Primarily Morriston Hospital with coverage at Singleton, Neath Port Talbot, and Gorseinon

Swansea City and County Council is seeking to recruit a qualified and experienced Adults Social Worker to join the Home First Team on a locum basis. This is a fantastic opportunity for a dedicated social worker looking to make a meaningful impact by supporting adults during critical transition periods from hospital to home.

The Home First Team plays a pivotal role in facilitating timely, safe and person-centred discharges from hospital settings back into the community. The team works collaboratively with health professionals, therapists, discharge coordinators and community partners to ensure individuals receive the right support at the right time. You will join a supportive, experienced group of professionals committed to strengths-based outcome-focused practice.

Role Overview:
  • Based primarily at Morriston Hospital with potential duties at Singleton, Neath Port Talbot, and Gorseinon.
  • Work closely with patients, families, health staff and care providers to plan and coordinate effective discharge pathways.
  • Undertake assessments in accordance with the Social Services and Wellbeing (Wales) Act 2014.
  • Ensure timely safeguarding and mental capacity assessments when appropriate.
  • Support the development of care and support plans that promote independence and reduce hospital readmission.
  • Manage a caseload in a fast-paced environment where effective decision-making and resilience are key.
What Swansea are looking for:
  • Registered with Social Care Wales (or eligible for registration).
  • Proven experience in hospital discharge or working within an acute setting.
  • Knowledge and practical application of the Social Services and Wellbeing (Wales) Act, Mental Capacity Act and safeguarding procedures.
  • Strong communication, assessment and decision-making skills.
  • Ability to work autonomously while contributing to a multidisciplinary team.
  • Available to start as soon as possible and able to work on-site five days per week (Monday–Friday).
