The role will be based in one building caring for residents in their own homes in line with agreed plans . The role will involve providing personal care to residents and also medication administration.

Due to the nature of this role, Domiciliary care experience(or similar setting experience) and previous experience in medication administration is required.

To provide personal care and housing related support to customers in accordance with agreed care and support plans, including accessing the local community.

Key tasks will involve:

Assist in the assessment and review of individual's care and support plans and ensure an excellent standard of service is delivered.

To ensure records are kept up to date in line with procedures.

To ensure performance targets, standards and objectives are met.

To undertake such other duties, training and/or hours of work as may be reasonably required.

Ensure that a high standard of customer care is provided at all times.

Treat people with dignity and respect.

Communicate effectively with customers and colleagues.

To ensure records are kept up to date in line with procedures.

Ensure Care Quality Commission Standards are met and evidenced.

To ensure that Swan's Equal Opportunities Policies are complied with and promoted in carrying out duties of the post.

To carry out any other duties and responsibilities commensurate with the post as required.
