Our service supports individuals experiencing Mental Health Crisis. The service is designed to prevent hospital admission by supporting individuals in the community in their own homes. It also supports with Hospital Discharge supporting individuals with the next step of their transition into the community.
Day to Day:
- Play a vital role in supporting the Community Crisis Support Service and the individuals who rely on it.
- Deliver short-term crisis care in people's homes, aiming to prevent unnecessary hospital or care home admissions and enabling individuals to remain safely in their own communities across Rushcliffe, Nottingham North East and West.
- Assess clients’ needs and create tailored care plans to meet them.
- Provide assistance with personal care, medication reminders, end-of-life care, and the preparation of light meals and drinks.
- Previous experience supporting individuals in Community Mental Health services.
- It is essential the applicant is a driver with use of a car and business insurance for service user visits.
- Ability to communicate with health and social care professionals.
- Transferable full enhanced DBS would be highly desirable.
- Good geographical knowledge of the service catchment area.
