Are you an experienced Mental Health Nurse or General Nurse seeking a new challenge? Are you looking for a post within Continuing Healthcare?

Service Care Solutions are working with Northamptonshire NHS to recruit a CHC Nurse Assessor on a temporary basis. The duration of the contract is for 6-months with the likelihood of extension. The successful candidate will be expected to work 37.5 hours per week across 5 days. Part-time applications will also be considered.

Shifts: Monday to Friday, 09:00-17:00

Pay: Negotiable Hourly Rate paid via an Umbrella LTD Company + £150 Sign-Up Bonus

Role: CHC Nurse Assessor

  • The post holder will be expected to be an effective member of the Continuing Healthcare Team, working flexibly to meet the changing needs of the health community.
  • To assist in the implementation and delivery of National Framework for NHS Continuing Healthcare, NHS Funded Nursing Care.
  • Provide Holistic and Individual reviews and initial assessments to patients within the resident populations as delegated by the Team Leader or caseload holder.


  • Must be a Registered Nurse registered with the NMC (RMN/RNLD/RGN)
  • Must have previous experience working in Continuing Healthcare
  • Must have a valid Driving License and Access to Own Method of Transport
