We are currently recruiting a SEND Key Worker to join the Education, Health & Care Assessment & Planning Team within Staffordshire County Council's Families First – Targeted Services. This is an exciting opportunity to be part of a dedicated team working in partnership with families and professionals to co-ordinate outcome-focused Education, Health and Care Plans (EHCPs).

About the Role: As a SEND Key Worker, you will play a central role in working collaboratively with children, young people and their families to assess needs, set achievable outcomes, and ensure access to the right support. You will coordinate input from a range of professionals – including social workers, health practitioners, and educational psychologists – to ensure joined-up, person-centred planning.

You will act as the main point of contact for the family throughout the EHCP process, helping to ensure services are tailored, inclusive, and responsive to individual needs.

Key Responsibilities:
  • Facilitate multi-agency assessments and develop outcome-based EHCPs

  • Build and maintain strong, respectful relationships with families

  • Coordinate support from internal and external professionals

  • Identify and manage risk in a solution-focused and strengths-based way

  • Record key information and progress using case management systems

  • Monitor and review plans, adapting support as necessary

  • Champion the voice and aspirations of the child or young person

About You: We’re looking for someone who is:

  • Experienced in working with children, young people or adults with SEND

  • Knowledgeable about child and adolescent development, disability, and SEND legislation

  • Skilled at building relationships, facilitating meetings and working with families

  • Highly organised and resilient, with a flexible and reflective approach

  • Committed to equality, safeguarding, and achieving positive outcomes

A relevant qualification (such as NVQ Level 3 or equivalent) and experience in education, social care, or a related field is essential.
