Liverpool City Council is seeking a dedicated and experienced Social Care Assessor to join our Adult Social Care & Health team on a part-time basis. This is a fantastic opportunity to contribute to a strengths-based, person-centred approach to social care with a focus on reablement.
Key Responsibilities:
- Carry out social care assessments and develop support plans that promote independence
- Work directly with individuals, carers, families, and partners to coordinate care
- Manage your own caseload and ensure timely reviews and updates to care plans
- Apply a reablement-focused approach to all interventions
- Use Liquid Logic to maintain accurate case records and reports
- Work collaboratively across multi-disciplinary teams to improve outcomes
- Experience working in a social care setting with a focus on reablement
- Confident and competent using Liquid Logic case management system
- Ability to deliver strength-based assessments and outcome-focused care plans
- Good understanding of adult social care legislation and safeguarding practice
- Strong communication, organisation, and partnership-working skills
