Social Care Assessor

Job Description

Service Care Solutions are recruiting for a Social Care Assessor to join Liverpool City Councils care line adults prevention and early intervention service team.

You will work within the adult service to identify what care is required for adults who are new to the service and also adults who's case has been closed.

This is based on a 3-month contract however, depending on performance this could increase or even become more permanent.

What you will do:

  • Undertake the assessment and review of social care and equipment needs for adults
  • Prioritise caseload with support and direction from a team manager
  • Ensure that the social care needs of individuals are identified and met

What you need:

  • Experience within the field of adult social care or relevant roles
  • Ability to gain relevant qualifications
  • Must be a car driver
