Responsibilities:
- Conduct assessments of children and young people with disabilities within the 0 - 25 age range
- Develop and implement care plans based on individual needs
- Provide support and guidance to families and caretakers
- Collaborate with other professionals to ensure holistic care for clients
- Maintain accurate records and documentation in accordance with regulations
