About the Role
As a Wellbeing Officer, you’ll play a vital frontline role in delivering all-age, community-based services that promote independence, wellbeing, and resilience. You’ll assess needs, develop support plans, and work face-to-face with individuals in their homes and neighbourhoods. These particular roles focus on supporting Carers in Doncaster—holding strengths-based conversations, completing Carer’s Assessments, and helping individuals access the support they need.
Key Responsibilities
- Deliver person-centred, all-age support to help individuals maintain health, wellbeing, and independence.
- Undertake holistic case management, including support with physical and mental health, loneliness, housing, finances, and safeguarding.
- Promote early intervention and prevention approaches across communities.
- Work in partnership with local services and organisations to build community capacity and deliver integrated support.
- Carry out Carer’s Conversations and develop tailored support plans based on individual needs.
- Identify gaps in local provision and help access or secure funding opportunities.
- Contribute to local and national programmes such as Stronger Families, Community Led Support, and the Adults Transformation Programme.
- Support Ward Members in their community leadership roles.
We’re seeking individuals who are:
- Experienced in community, health, or social care roles, especially within public sector settings.
- Knowledgeable about wellbeing strategies, the Care Act, and early intervention models.
- Confident communicators with the ability to engage sensitively with vulnerable individuals.
- Proactive, empathetic, and organised, with a creative approach to problem-solving.
- Committed to promoting independence and enabling people to do more for themselves.
- Strong interpersonal and influencing skills
- Casework planning and assessment experience
- Ability to manage a varied and dynamic workload
- Creative thinking and community development experience
- Understanding of carers’ needs and community wellbeing initiatives
