We are seeking an experienced CHC Social Worker (Adults) (Grade 10) Day Rate - Social Care Qualified to join a dedicated team on a short-term contract for 2 weeks. This is an excellent opportunity for a Social Care Qualified Social Worker who can hit the ground running and deliver high-quality, needs-led support for adult clients.

Candidate Responsibilities

As a CHC Social Worker (Adults) (Grade 10) Day Rate - Social Care Qualified, you will:

• Complete CHC assessments and contribute to care planning and evidence-based decision making
• Conduct safeguarding-focused reviews, ensuring appropriate actions are taken promptly
• Maintain accurate case notes, documentation, and activity records to required standards
• Liaise with multi-disciplinary teams, stakeholders, and partner agencies
• Support service users to access the right services, ensuring outcomes are monitored and reviewed
• Ensure compliance with local policies, relevant legislation, and professional standards
