Adult North Team - Adult Social Worker (Adult SW) – Contract Opportunity

Are you an experienced Social Worker looking for a rewarding temporary role in our client? We are currently seeking a dedicated Adult North Team - Adult SW for a 2-week contract, offering flexible hybrid working arrangements and the chance to make a meaningful impact within a supportive team environment.

About the Role:

As an Adult North Team - Adult SW, your responsibilities will include working within a small, dynamic team based in North our client. You will be instrumental in conducting Care Act assessments, reviews, CHC checklists, Discharge to Assess (DTA), and Care of the Person (COP) work as needed. This role involves participation in a duty rota, ensuring that clients’ needs are met efficiently and compassionately.

Candidate Responsibilities:

  • Conduct Care Act assessments, reviews, and relevant assessments
  • Complete CHC checklists, DSTs, and COP work where appropriate
  • Participate in a duty rota covering office and community visits
  • Work collaboratively with team members to ensure high standards of social care
  • Maintain accurate and up-to-date case records and documentation
  • Comply with all relevant safeguarding policies and procedures

Candidate Requirements:

  • Proven experience as a Social Worker, preferably within adult social care
  • HCPC registration is mandatory
  • Experience in completing assessments, reviews, and care planning
  • Ability to work effectively as part of a team and independently
  • Flexibility to work in a hybrid setting (minimum 2 days in the office)
  • Comprehensive understanding of the Care Act and related legislation
