• £36 per hour umbrella
• £31.66 PAYE (inclusive)
• £28.25 PAYE (exclusive)

Job Overview Service Care Solutions are recruiting an experienced Adult Social Worker to join a Hospital Discharge Team within a Local Authority based in Kendal. The role focuses on supporting timely and safe discharges from hospital, working with adults with varying levels of need in accordance with statutory duties.
This is a fast-paced role requiring strong assessment skills and experience within adult services.

Key Responsibilities

• Complete assessments under the Care Act 2014 to support safe hospital discharge
• Work with multidisciplinary professionals including health, therapy, and discharge teams
• Plan and coordinate discharges to ensure appropriate care and support is in place
• Manage a caseload of adults with varying levels of need
• Complete risk assessments and safeguarding interventions where required
• Arrange care packages, placements, and ongoing community support
• Maintain accurate, timely, and high-quality case records
• Participate in multidisciplinary meetings and reviews
• Ensure practice is person-centred and outcome focused

Essential Requirements

• Qualified Social Worker
• Registered with Social Work England
• Recent experience within Adult Social Care, ideally within a discharge or hospital setting
• Sound knowledge of the Care Act 2014
• Strong assessment, risk management, and decision-making skills
• Ability to work effectively in a fast-paced, multidisciplinary environment
• Enhanced DBS (Update Service desirable)
