Rate: £43.00 per hour Umbrella (£33.68 PAYE)

Service Care Solutions are recruiting for an experienced Adults Social Worker (Level 2) to join our client Physical Disability and Older People’s Team in Norwich.

About the Role

The team operates within the principles of Community Led Support (CLS), focusing on meaningful conversations, early intervention, and creative community-based solutions that promote independence and wellbeing.

Key responsibilities include:

• Managing a varied community caseload involving adults with physical disabilities and older people
• Undertaking Care Act assessments and reviews
• Conducting Mental Capacity Act assessments
• Completing safeguarding enquiries
• Producing outcome-focused support plans
• Working collaboratively with health professionals and partner agencies
• Delivering timely and proportionate interventions to achieve positive outcomes

Requirements

• Social Work England registration
• Qualified Social Worker status
• Experience working with adults with physical disabilities and older people
• Sound knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures
• Experience managing complex community caseloads
• Strong assessment, communication, and organisational skills
• Ability to work effectively within a multidisciplinary team
• Previous experience using our client systems would be advantageous
