Adult Social Worker – Targeted Review Team
Location: Medway
Rate: £38 per hour (Umbrella)
Contract: 3 Months (Ongoing)
Working Pattern: Hybrid (3 days office / 2 days home)

Service Care Solutions are recruiting for an Adult Social Worker to join the Targeted Review Team within a local authority in Medway.

Key Responsibilities:

• Manage a mixed caseload within a community social work team
• Undertake strengths-based assessments and develop person-centred care plans
• Carry out face-to-face visits and interventions
• Support adults, families, and carers to promote independence and wellbeing
• Work in line with the Care Act, Mental Capacity Act, and safeguarding procedures
• Collaborate with multi-agency partners to deliver effective outcomes
• Maintain accurate case records and prepare professional reports

Requirements:

• Qualified Social Worker with Social Work England registration
• Completed ASYE
• Experience working with adults in a statutory setting
• Knowledge of relevant legislation (Care Act, MCA, etc.)
• Strong assessment and case management skills
