💷 PAYE: £26.55 per hour
💷 Limited/Umbrella: £34.72 per hour

About the Role
Service Care Solutions are currently recruiting for an experienced Adults Social Worker – Disabilities to join the South Disabilities Operational Team at Powys County Council.

This is an excellent opportunity for a qualified Social Worker with experience in Adults Social Care and/or Disabilities Services to join an established operational team and contribute to positive outcomes for adults and their families.

The role offers flexible working arrangements, providing a good balance between professional responsibilities and flexibility.

Key Responsibilities
As an Adults Social Worker within the Disabilities team, you will:

• Undertake assessments of adults with care and support needs.
• Develop and review person-centred support plans.
• Work alongside individuals, families and carers to identify strengths, needs and desired outcomes.
• Safeguard adults and respond appropriately to safeguarding concerns.
• Work collaboratively with health professionals, providers and other partner agencies.
• Apply relevant legislation, policies and professional standards to social work practice.
• Maintain accurate and timely case records and documentation.
• Support individuals to maximise independence, choice and control.
• Participate in reviews and ensure appropriate care and support arrangements are in place.

About You
The successful candidate will be a Social Care Wales registered and qualified Social Worker with relevant experience within Adults Social Care.
You will ideally have:

• A recognised Social Work qualification.
• Current Social Care Wales registration.
• Experience working within Adults Social Care.
• Experience working with adults with disabilities is highly desirable.
• Knowledge of relevant legislation and safeguarding procedures.
• Strong assessment, care planning and review skills.
• Excellent communication and interpersonal skills.
• The ability to work effectively both independently and as part of a multidisciplinary team.
