Are you an experienced Adults Social Worker seeking a rewarding locum opportunity? Service Care Solutions is recruiting for an Adults Social Worker to join the Physical Disability & Sensory Impairment Team at Caerphilly County Borough Council. Key Responsibilities of the Adults Social Worker - Physical Disabilities
As an Adults Social Worker - Physical Disabilities, you will be responsible for:

• Managing a diverse caseload involving adults with physical disabilities and sensory impairments
• Completing strengths-based assessments and reviews
• Undertaking Care Management in line with statutory duties and best practices
• Developing, implementing, and reviewing person-centred care and support plans
• Carrying out risk assessments and safeguarding responsibilities when necessary
• Collaborating effectively with multidisciplinary teams, including health professionals and partner agencies
• Responding appropriately to crisis situations and changing service user needs
• Maintaining accurate case records, reports, and documentation
• Participating in supervision, team meetings, and professional development activities
• Contributing to service development and promoting best social work practices

Candidate Requirements
To be considered for the Adults Social Worker - Physical Disabilities role, you must have:

• Degree or Diploma in Social Work (or equivalent qualification)
• Registration with Social Care Wales (or an equivalent UK Social Work regulator)
• Proven experience within Adult Social Care, including assessment and care planning
• Knowledge of the Social Services and Well-being (Wales) Act, Mental Capacity Act, Mental Health Act, and Deprivation of Liberty Safeguards
• Strong communication, assessment, and decision-making skills
• Ability to work independently and within multidisciplinary teams
• Full UK Category B driving licence and access to a vehicle insured for business use
