The Role

Key Responsibilities
- Complete assessments and reviews of adults with physical disabilities and sensory impairments.
- Undertake Care Management in accordance with statutory duties and best practice.
- Develop, implement, and review person-centred care and support plans.
- Carry out risk assessments and safeguarding responsibilities where required.
- Work effectively within multidisciplinary teams, including Health and Social Care professionals.
- Respond appropriately to crisis situations and changing service user needs.
- Maintain accurate case records, reports, and documentation.
- Participate in supervision, professional development, and team meetings.
- Contribute to service development and the promotion of best social work practice.
To be considered for this role, applicants must have:
- Degree or Diploma in Social Work (or equivalent qualification).
- Registration with Social Care Wales (or an equivalent UK Social Work regulator).
- Experience within Adult Social Care, including assessment and care planning.
- Knowledge of the Social Services and Well-being (Wales) Act, Mental Capacity Act, Mental Health Act, and Deprivation of Liberty Safeguards.
- Strong communication, assessment, and decision-making skills.
- Ability to work independently and collaboratively within multidisciplinary teams.
- Full UK Category B driving licence and access to a vehicle insured for business use.
- Competitive hourly rates of £30.00 PAYE or £39.58 Umbrella.
