Working Pattern: Hybrid (Average 2 days remote, 3 days office-based. Office attendance required when on duty.)
Pay Rate: £30.00 per hour PAYE / £39.58 per hour Umbrella

Service Care Solutions are currently recruiting for an experienced Adults Social Worker to join the Physical Disability & Sensory Impairment Team at Caerphilly County Borough Council.

This is an excellent opportunity for a qualified Social Worker looking to join a supportive local authority, delivering high-quality services that promote independence, wellbeing, and positive outcomes for adults with physical disabilities and sensory impairments.

The Role
As an Adults Social Worker, you will manage a varied caseload, undertaking strengths-based assessments, care management, safeguarding, and reviews. You will work collaboratively with health professionals and partner agencies to ensure service users receive the appropriate support to meet their assessed needs.

Key Responsibilities

• Complete assessments and reviews of adults with physical disabilities and sensory impairments.
• Undertake Care Management in accordance with statutory duties and best practice.
• Develop, implement, and review person-centred care and support plans.
• Carry out risk assessments and safeguarding responsibilities where required.
• Work effectively within multidisciplinary teams, including Health and Social Care professionals.
• Respond appropriately to crisis situations and changing service user needs.
• Maintain accurate case records, reports, and documentation.
• Participate in supervision, professional development, and team meetings.
• Contribute to service development and the promotion of best social work practice.

Requirements
To be considered for this role, applicants must have:

• Degree or Diploma in Social Work (or equivalent qualification).
• Registration with Social Care Wales (or an equivalent UK Social Work regulator).
• Experience within Adult Social Care, including assessment and care planning.
• Knowledge of the Social Services and Well-being (Wales) Act, Mental Capacity Act, Mental Health Act, and Deprivation of Liberty Safeguards.
• Strong communication, assessment, and decision-making skills.
• Ability to work independently and collaboratively within multidisciplinary teams.
• Full UK Category B driving licence and access to a vehicle insured for business use.
