Location: Taunton, Somerset
Pay Rate: £44.00 per hour Umbrella / £32.13 per hour PAYE
Working Pattern: Hybrid Working - 1 day per week in the office (Wednesday at County Hall, Taunton)

Service Care Solutions are currently recruiting for an experienced Advanced Practitioner Social Worker to join the Safeguarding Team within Adult Social Care in Somerset.

This is an excellent opportunity for a skilled practitioner to provide professional leadership and deliver high-quality social work services that promote independence, wellbeing, and positive outcomes for adults across the community.

Key Responsibilities

• Provide professional leadership and support within Adult Social Care services.
• Promote wellbeing, independence, and person-centred practice in line with the Care Act.
• Complete timely and robust assessments, reviews, and support plans.
• Safeguard vulnerable adults and manage complex safeguarding enquiries.
• Support individuals to remain independent within their communities for as long as possible.
• Work collaboratively with health professionals, families, carers, and partner agencies.
• Facilitate safe and timely hospital discharges and help prevent unnecessary admissions.
• Ensure services enhance existing informal support networks and community resources.
• Support people to achieve outcomes that are important to them while maintaining choice and control over their care and support.

Requirements

• Qualified Social Worker.
• Registered with Social Work England.
• Significant experience working within Adult Social Care.
• Strong knowledge of safeguarding legislation, policy, and practice.
• Experience managing complex cases and undertaking risk assessments.
• Excellent communication, decision-making, and leadership skills.
