Agency Experienced Hospital Social Worker (Contract)

Are you an experienced social worker with a passion for helping patients transition smoothly from hospital to home? We are currently seeking Agency Experienced Hospital Social Workers to join the Transfer of Care Hub at our client Hospital on a temporary contract basis. This is a fantastic opportunity to work in a fast-paced, dynamic environment committed to ensuring safe, timely discharges.

Position Details:

  • Job Title: Agency Experienced Hospital Social Worker
  • Employment Type: Contract (2 weeks)
  • Location: our client Hospital (hybrid working – three days in the office each week)
  • Working Hours: Full-time, 37 hours per week

Candidate Responsibilities:
As an Agency Experienced Hospital Social Worker, your main responsibilities will include:

  • Conducting comprehensive assessments to determine patients' social care needs
  • Collaborating with multidisciplinary teams to facilitate safe and efficient patient discharges
  • Supporting patients and families through the discharge process, ensuring all needs are met
  • Reducing delays in transfers of care by proactively managing case workloads
  • Maintaining accurate documentation and adhering to safeguarding procedures
  • Working within relevant social care and hospital protocols, including knowledge of the Mosaic social care system (desirable)

Candidate Requirements:
To succeed as an Agency Experienced Hospital Social Worker, you should possess:

  • Recent experience in hospital or Transfer of Care settings (essential)
  • Strong assessment, communication, and decision-making skills
  • Ability to work effectively in a fast-paced environment
  • Familiarity with social care systems is desirable, particularly Mosaic
  • At least 3 years of relevant social work experience supported by referees
  • Ability to commit to the full 2-week contract
