We are currently seeking Agency Experienced Hospital Social Workers to join the Transfer of Care Hub at Warrington Hospital on a temporary contract basis. This is a fantastic opportunity to work in a fast-paced, dynamic environment committed to ensuring safe, timely discharges.

Position Details:

• Job Title: Agency Experienced Hospital Social Worker
• Employment Type: Contract (2 weeks)
• Location: Warrington Hospital (hybrid working – three days in the office each week)
• Working Hours: Full-time, 37 hours per week

Candidate Responsibilities:
As an Agency Experienced Hospital Social Worker, your main responsibilities will include:

• Conducting comprehensive assessments to determine patients' social care needs
• Collaborating with multidisciplinary teams to facilitate safe and efficient patient discharges
• Supporting patients and families through the discharge process, ensuring all needs are met
• Reducing delays in transfers of care by proactively managing case workloads
• Maintaining accurate documentation and adhering to safeguarding procedures
• Working within relevant social care and hospital protocols, including knowledge of the Mosaic social care system (desirable)

Candidate Requirements:
To succeed as an Agency Experienced Hospital Social Worker, you should possess:

• Recent experience in hospital or Transfer of Care settings (essential)
• Strong assessment, communication, and decision-making skills
• Ability to work effectively in a fast-paced environment
• Familiarity with social care systems is desirable, particularly Mosaic
• At least 3 years of relevant social work experience supported by referees
• Ability to commit to the full 2-week contract
