Are you an experienced Social Worker (Adults) looking for a dynamic contract opportunity? We are seeking a dedicated and knowledgeable Social Worker (Adults) – Grade I to join our team for an initial 2-week assignment. This is an excellent chance to contribute your expertise within a collaborative environment and make a positive impact on adults in need of support.
Key Responsibilities of the Social Worker (Adults) – Grade I
As an Experienced Social Worker (Adults) – Grade I, your primary duties will include:
- Conducting assessments to determine clients’ needs and support plans
- Working closely with multi-agency teams to coordinate care and services
- Demonstrating strong knowledge of D2A (Discharge to Assess) processes and hospital discharge procedures
- Managing cases involving Continuing Healthcare (CHC) eligibility
- Engaging in face-to-face work, with a minimum of 3 days per week dedicated to direct client contact
- Maintaining accurate documentation in line with statutory and agency requirements
- Travel within and outside the council area as required, with possession of a full driving licence and access to a vehicle
Candidate Requirements for the Social Worker (Adults) – Grade I Role
The ideal candidate will possess:
- Proven experience as a Social Worker (Adults), particularly in assessment work
- Good understanding of D2A and hospital social work processes
- Strong multi-agency collaboration experience
- Knowledge of CHC guidelines and processes
- A full UK driving licence and access to a vehicle
- Willingness to travel both inside and outside the council area
