Hospital Discharge Social Worker – Middleton Locality

Are you an experienced and passionate Hospital Discharge Social Worker looking for an exciting contract opportunity? Our client Adult Care, rated GOOD by the CQC, is seeking a dedicated Hospital Discharge Social Worker to join our dynamic Home First Hospital Discharge Hub based within the Middleton Locality. This is a fantastic chance to make a meaningful difference in adults’ lives by supporting safe and timely hospital discharges.

Position Details

  • Job Title: Hospital Discharge Social Worker – Middleton Locality
  • Employment Type: Contract (2 weeks)
  • Location: Middleton, our client (with travel to hospital sites in our client and beyond)
  • Pay Rate: Competitive, to be discussed during application

Candidate Responsibilities
As a Hospital Discharge Social Worker, your key responsibilities will include:

  • Managing hospital discharge planning for adult patients, ensuring timely and safe transitions from hospital to home or other suitable settings
  • Conducting comprehensive assessments under the Mental Capacity Act, including Mental Capacity Assessments and Best Interest processes
  • Developing innovative, solution-focused care plans that promote independence and meet discharge needs in line with the Care Act 2014
  • Working collaboratively with health colleagues, district nurses, carers, and voluntary agencies to deliver person-centred support
  • Promoting strength-based approaches that endorse service users’ independence and well-being
  • Maintaining accurate records, documenting assessments, actions, and outcomes using electronic and manual systems
  • Participating in duty and team meetings, supporting flexible working and out-of-hours requirements

Candidate Skills and Experience
The ideal Hospital Discharge Social Worker will possess:

  • Current registration with Social Work England
  • Proven hospital social work experience within adult care settings
  • In-depth knowledge of the Mental Capacity Act and practical experience in assessments and Best Interests processes
  • Excellent communication and interpersonal skills, with the ability to build positive relationships quickly
  • Strong organisational skills and the ability to multi-task efficiently
  • Proficiency with IT packages including Word, Excel, and Outlook
  • A proactive, motivated, and forward-thinking attitude
  • A valid driver’s license and access to a car, as visiting hospital sites locally and out of area is essential
