Reablement Occupational Therapist – our client

Service Care Solutions are recruiting for a Reablement Occupational Therapist to work with Our client on a temporary contract.

Location: Our client
Contract: 3-month ongoing
Hours: Monday to Friday, 9:00am – 5:00pm (35 hours per week)
Rate: £35.38 per hour (Umbrella) / £27.77 per hour (PAYE)

Role Overview

This role focuses on supporting individuals through short-term reablement interventions, helping them regain independence and remain safely in their own homes following illness, injury, or hospital discharge.

Key Responsibilities

• Carry out functional assessments to support reablement goals
• Develop and implement tailored care and rehabilitation plans
• Provide and recommend equipment and minor adaptations
• Work closely with reablement teams, care staff, and multidisciplinary professionals
• Support timely hospital discharges and prevent readmissions
• Monitor progress and adjust interventions to maximise independence
• Deliver advice and guidance to service users, families, and carers

Requirements

• Qualified Occupational Therapist
• HCPC registered
• Experience within reablement, community, or adult social care settings
• Strong assessment and communication skills
