Are you an experienced Occupational Therapist looking for an exciting opportunity to make a positive impact in the community? Service Care Solutions is currently seeking a dedicated Occupational Therapist Reablement to join a reablement service based in Chippenham, Wiltshire. This is a fantastic chance to support adults in regaining independence and promoting safe living at home following illness, injury, or hospital discharge.

Job Title: Occupational Therapist Reablement

Contract Duration: 2 weeks (with potential for extension)
Location: Chippenham, Wiltshire
Working Hours: Full-Time
Rate of Pay:

• Umbrella: £36.00 per hour
• PAYE: £28.78 per hour

Candidate Responsibilities:

• Conduct Occupational Therapy assessments within community and reablement settings.
• Support adults to enhance independence and achieve personal goals through tailored interventions.
• Recommend appropriate equipment, adaptations, and safe moving and handling solutions.
• Develop and review reablement plans to facilitate effective discharge from hospital and reduce unnecessary admissions.
• Collaborate with multidisciplinary teams, carers, and community services to deliver comprehensive care and positive outcomes.

Required Skills & Qualifications:

• Degree or Diploma in Occupational Therapy
• HCPC Registration
• Previous experience within Reablement, Community OT, Adult Social Care, or Intermediate Care services
• Strong assessment and care planning skills
• Full UK Driving Licence and access to a vehicle preferred
