Social Worker – Home First (Hospital Team)

£38.43 Umbrella (£30.50 PAYE) per hour
Monday to Friday | Full-Time | On-Site

Service Care Solutions are currently recruiting for an experienced Social Worker to join our client within Swansea Local Authority, working as part of a fast-paced and dedicated Home First Hospital Team.
This is a full-time position (Monday to Friday) with an initial 2–3 month ongoing contract, with a strong likelihood of extension.

The Role: You will support individuals to safely transition from hospital back into the community, promoting independence and reducing unnecessary hospital stays.
Key responsibilities include:

• Completing Care Act assessments and support planning
• Supporting safe and timely hospital discharges
• Working within a multi-disciplinary team (MDT)
• Conducting Mental Capacity Assessments and Best Interest decisions
• Liaising with families, carers, and external agencies
• Managing a complex and fast-paced caseload
• Responding to safeguarding concerns

Requirements:

• Must be registered with Social Care Wales
• Experience within a hospital discharge, D2A or Home First setting
• Strong knowledge of Care Act, MCA, and safeguarding legislation
• Ability to work effectively in a high-pressure environment

Contract Details:

• £38.43 per hour Umbrella / £30.50 per hour PAYE (exclusive)
• Full-time – Monday to Friday
• Initial 2–3 month contract
• Extension likely
• Fully on-site role – no hybrid working

Bonuses:

• £200 referral bonus for a successful candidate
