Social Worker – Adults Inpatient Team (Norfolk) Service Care Solutions
Service Care Solutions are currently recruiting for a Social Worker to work with adults within an Inpatient Team for a local authority client in Norfolk, based at the Norfolk and Norwich University Hospital.
Contract: 6-month ongoing
Hours: Monday to Friday, 9:00am – 5:30pm
Pay Rate: £44.65 umbrella / £34.96 PAYE
The Role This is an exciting opportunity to join a dedicated multidisciplinary team, supporting adults during inpatient hospital stays. You will play a key role in facilitating safe, timely discharges, ensuring individuals receive the right support at the right time.
Working within a Home First model, you will promote independence and recovery through strong partnership working across health and social care services. The role will also involve reviewing individuals post-discharge, whether in their own homes or within care settings.

Key Responsibilities
- Undertake person-centred assessments with patients and their families
- Develop and implement care plans that promote independence and wellbeing
- Work collaboratively with multi-disciplinary teams to ensure smooth discharge planning
- Support individuals post-discharge through reviews in the community or care placements
- A strong understanding of the Care Act 2014
- Experience completing Mental Capacity Assessments and Best Interest decisions
- Knowledge and experience of Continuing Healthcare (CHC) processes
- Excellent communication and organisational skills
- The ability to work effectively in a fast-paced hospital environment
