Social Worker - 34 (Adults) - Contract

Are you an experienced Social Worker looking to make a meaningful difference in the community? We are currently seeking a dedicated Social Worker - 34 (Adults) to join our Physical Disability and Older People’s Team in Norwich on a 3-month contract. This is an excellent opportunity to work within a forward-thinking, supportive service committed to delivering high-quality, strengths-based practice for adults across the community.

About the Role

As a Social Worker - 34 (Adults), you will be an integral part of a team that values relational practice and collaboration. Your responsibilities will include managing a varied community caseload, handling complex safeguarding cases, conducting Mental Capacity Act assessments, and fulfilling Care Act duties. You will contribute to timely, outcome-focused assessments and planning, ensuring individuals receive the support they need to remain independent and resilient.

This role operates within the principles of Community Led Support (CLS), emphasizing meaningful conversations, early intervention, and innovative, community-based solutions. You will work closely with partners across health and social care to achieve the best possible outcomes for service users.

Candidate Responsibilities for the Social Worker - 34 (Adults):

  • Manage a diverse caseload of adults with physical disabilities and older people
  • Conduct safeguarding investigations and reviews
  • Perform Mental Capacity Act assessments and best interest decisions
  • Carry out assessments under the Care Act and develop outcome-focused support plans
  • Engage in meaningful conversations with service users and their families
  • Work collaboratively with multidisciplinary teams and external partners
  • Maintain accurate and timely case documentation in line with organisational policies
  • Uphold professional standards and continually reflect on practice

Candidate Requirements

  • Qualified Social Worker registered with Social Work England
  • Significant experience working with adults in the community, including those with physical disabilities and older people
  • Strong organisational and time-management skills
  • Sound professional judgment and decision-making capabilities
  • Able to work effectively within multidisciplinary teams
  • Familiarity with Norfolk County Council systems is highly advantageous
