Job Overview

The Social Worker - 34 (Adults) position is based within the Physical Disability and Older People’s Team, focusing on providing high-quality, strengths-based social care. The role involves managing a varied community caseload, including complex safeguarding cases, assessments under the Mental Capacity Act, and Care Act responsibilities. If you are registered with Social Work England and have experience working with adults in community settings, this contract offers an engaging environment to develop your skills while making a real difference.

Candidate Responsibilities

As a Social Worker - 34 (Adults), your key responsibilities will include:

• Managing a diverse community caseload involving adults with physical disabilities and older people
• Conducting safeguarding enquiries and assessments under the Mental Capacity Act and the Care Act
• Delivering timely, proportionate assessments and outcome-focused care planning
• Engaging in meaningful conversations and early interventions aligned with Community Led Support (CLS) principles
• Working collaboratively with multidisciplinary teams and external partners to achieve the best outcomes for clients
• Ensuring compliance with all relevant policies and procedures, including our client systems and processes
• Demonstrating sound professional judgment and strong organisational skills throughout all cases

Candidate Requirements

• Relevant Social Work qualification and registration with Social Work England
• Proven experience managing community caseloads involving adults with disabilities and older adults
• Comfortable working within a multidisciplinary team environment
• Strong organisational and communication skills
• Knowledge of our client systems and processes (desirable)
