Social Worker – Children and Young People Disability TeamService Care Solutions are recruiting on behalf of Medway Council for an experienced Social Worker to join their Children and Young People Disability Team.

This is an excellent opportunity to work within a specialist service supporting disabled children, young people and their families. You'll manage a varied caseload, delivering high-quality assessments, safeguarding interventions and transition planning while making a real difference to the lives of vulnerable children and young adults.

The Role
As a Social Worker within the Children and Young People Disability Team, you will:

• Manage a caseload of disabled children and young people with appropriate reflective supervision.
• Complete high-quality, analytical assessments and develop SMART care plans and reviews.
• Deliver relationship and strengths-based practice, building positive partnerships with children and families.
• Undertake direct work with children and young people, ensuring their wishes and lived experiences are central to decision-making.
• Use the Signs of Safety practice model in assessments and interventions.
• Prepare high-quality reports for Child Protection Conferences, Children in Need meetings, Looked After Children Reviews, strategy meetings and court proceedings.
• Maintain accurate case records using Medway Council's electronic recording systems.
• Participate in safeguarding duties, including duty cover and supporting the Multi-Agency Safeguarding Hub (MASH).
• Work collaboratively with education, health, police and partner agencies to achieve positive outcomes.
• Maintain continuous professional development and uphold Social Work England professional standards.

About You
To be considered, you will have:

• Qualified Social Worker status.
• Successful completion of your ASYE.
• Registration with Social Work England.
• Experience working with disabled children, young people and their families.
• Experience within statutory Children's Social Care.
• Knowledge of child protection, safeguarding and court work.
• Strong assessment, report writing and analytical skills.
• Experience delivering relationship and strengths-based practice.
• Excellent communication and partnership working skills.
• A full UK driving licence and access to a vehicle for business use.
• Commitment to reflective supervision and continuous professional development.

Desirable

• Experience using the Signs of Safety practice model (or willingness to undertake training).
• Experience using Mosaic or a similar electronic case management system.
• Knowledge of current legislation, statutory guidance and best practice relating to disabled children and young people.
