Social Worker – Adults Inpatient Team (Norfolk) | Contract (2 weeks)

Service Care Solutions is recruiting a Social Worker to join an Adults Inpatient Team for a local authority client in Norfolk, based at the Norfolk and Norwich University Hospital. This Social Worker contract is for 2 weeks, with an indicative assignment framework of 6 months ongoing, Monday to Friday 9:00am – 5:30pm.

Pay Rate for Social Worker

• £44.65 per hour (umbrella)
• £34.96 per hour (PAYE)

The Social Worker role

As a Social Worker, you will work within a dedicated multidisciplinary team supporting adults during inpatient hospital stays. You’ll help facilitate safe, timely discharges using a Home First approach, promoting independence and recovery through strong partnership working across health and social care services. You will also review outcomes post-discharge, whether individuals return home or transition into care settings.

Candidate Responsibilities

• Undertake person-centred assessments with patients and their families as a Social Worker
• Develop and implement care plans focused on independence and wellbeing as a Social Worker
• Collaborate with multi-disciplinary teams to support smooth discharge planning as a Social Worker
• Support individuals post-discharge through reviews in the community or care placements as a Social Worker
• Apply knowledge of the Care Act 2014 in day-to-day practice
• Complete Mental Capacity Assessments and Best Interest decisions
• Contribute to Continuing Healthcare (CHC) processes
