Social Worker – Adults Learning Disabilities (Community Care) | London (Merton)

Service Care Solutions is recruiting for a Social Worker to join the Adults Learning Disabilities Community Care Team in the London Borough of Merton. This is a temporary contract for 2 weeks, with strong extension potential. The role is hybrid (3 days office / 2 days home), working Monday–Friday, 9:00–17:00.

Pay rate

• £32 per hour Umbrella ( £24 PAYE )

Candidate Responsibilities

As a Social Worker, you will be responsible for:

• Undertaking the full care management process in line with the Care Act and statutory guidance
• Managing a caseload allocated by the Team Manager, ensuring accurate and up-to-date records (paper and IT systems)
• Completing risk assessments in line with Merton Care Management Procedures
• Delivering duties under relevant legislation, including Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DoLS)
• Supporting safeguarding enquiries and promoting Making Safeguarding Personal
• Contributing to commissioning/coordination of care packages, ensuring value for money and quality service delivery
• Maintaining accurate records on CareFirst and other systems
• Attending meetings, court hearings, and tribunals as required
• Supporting joint working with partner agencies and stakeholders
• Supporting service development, data reporting, and continuous improvement
