Job Title: Band 6 Continuing Healthcare (CHC) Nurse – Agency (Mid Essex ICB)
Location: Mid Essex
Pay Rate: £27 per hour (LTD) / £24.29 PAYE inclusive
Contract Type: Agency/Temporary
Working Hours: Full-time or part-time hours available; Monday to Friday, 9am – 5pm (some flexibility may be required)
Job Summary:
We are seeking an experienced Band 6 Continuing Healthcare (CHC) Nurse to support Mid Essex ICB on an agency basis. The successful candidate will be responsible for delivering high-quality CHC assessments and reviews, working within the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care. This is a great opportunity to work autonomously in a flexible, well-supported environment.
Key Responsibilities:
  • Complete comprehensive Continuing Healthcare (CHC) assessments using the Decision Support Tool (DST)
  • Undertake CHC and Funded Nursing Care (FNC) checklists, fast-track assessments, and 3/12 reviews
  • Attend and contribute to multidisciplinary team (MDT) meetings and provide eligibility recommendations
  • Liaise with patients, families, care providers, and other professionals to gather and review clinical evidence
  • Submit accurate documentation and reports within agreed timeframes
  • Stay updated on changes to the National Framework and apply them to practice
  • Manage own caseload independently while adhering to Mid Essex ICB’s quality and governance standards
Requirements:
  • Registered Nurse (RGN, RMN, RNLD) with active NMC registration
  • Minimum of 2 years’ experience in CHC assessments
  • Solid understanding and practical application of the National Framework for NHS CHC
  • Confident in using the Decision Support Tool and completing assessments autonomously
  • Enhanced DBS on the update service (or willingness to obtain one)
  • Ability to work remotely and travel locally if required
Desirable:
  • Prior experience working with an ICB, CCG, or CHC team (preferably within Essex)
  • Knowledge of systems such as Broadcare or CareTrack
  • Own transport for community visits (if needed)
