Job Description

Care Leaver Outreach Support Worker needed with Worcestershire County Council

Start Date- ASAP

Location - Worcester

Duration - Initially 3 months

Hours per week - 37 hours per week

Salary - £14.90 ltd an hour


  • To support young people who have left care who may present with issues where they require additional support
  • Support Care Leavers aged 18-24 years old
  • Provide assessment and support to young people who are looking to transition from residential care or foster care and/or who are identified as needing a high level of support to either maintain accommodation or work towards independence
  • Provide support for young people, participating in the assessment of their needs and the implementation of plans for their future
  • Provide assessment and support care leavers who may be homeless or at risk of becoming homeless
  • To provide support in emergency and crisis situations

What is required?

  • Significant experience of working with Young People as part of a team
  • Significant experience of direct work with Young People
  • Experience within a similar role
  • Level of education equivalent to Diploma 3 in a relevant field or equivalent experience
  • Drivers license and access to a vehicle
