Experienced Adult Social Worker
Service Care Solutions are seeking an experienced Adult Social Worker to join Liverpool City Council's Transitions Team. As an Adult Social Worker, you will be responsible for providing support and assistance to adults in need of care and support. You will work closely with other professionals to ensure that the needs of the service users are met.
Key Responsibilities
- Provide support to adults in need of care and support
- Assess the needs of service users and develop care plans
- Work closely with other professionals to ensure that the needs of service users are met
- Monitor and review the progress of service users
- Provide advice and guidance to service users and their families
Key Requirements
- A degree in Social Work
- Registration with Social Work England
- Experience working as an Adult Social Worker
- Knowledge of relevant legislation and policies
- Excellent communication and interpersonal skills
