Locum Adult Social Worker – Continuing Health Care Project
Newcastle City Council

Overview:
Newcastle City Council is seeking an experienced and motivated Locum Adult Social Worker to support a dedicated 6-month project focused on providing additional capacity to review Continuing Health Care cases. This is a critical role contributing to high-quality service delivery and timely decision-making for adults with complex health and care needs.

Key Responsibilities:
  • Plan and participate in Decision Support Tool (DST) / Multi-Disciplinary Team (MDT) meetings.
  • Coordinate and confirm attendance of relevant parties at DST meetings.
  • Prepare for DST meetings by gathering relevant case information from Eclipse (case management system), care providers and the individual/family.
  • Attend DST meetings and document outcomes accordingly:
    • If Primary Health Need (PHN) is agreed, record outcome on Eclipse – No Further Action.
    • If PHN is not agreed and disagreement exists:
      • Formally communicate disagreements to the Integrated Care Board (ICB).
      • Attend reconvened DSTs if necessary.
      • Draft dispute letters for Service Manager submission where required.
    • If PHN not agreed and no disagreement:
      • Complete Care Act Assessment, Support Plan and financial assessment request.
      • Facilitate transfer of case and funding responsibility to Newcastle City Council.
      • Conduct follow-up calls with the individual/family to explain the outcome and next steps.
Requirements:
  • Social Work qualification and registration with Social Work England.
  • Proven experience in adult social care - ideally including Continuing Health Care or Care Act assessments.
  • Familiarity with Eclipse or similar case management systems.
  • Strong understanding of CHC framework, Care Act legislation and dispute resolution processes.
  • Excellent communication, coordination and decision-making skills.
