Social Worker - Community Team - Worthing

Working arrangements - Hybrid working, with 50/50 split working from home and undertaking visits

Rate of pay - Up to £38ltd per hour or £35ltd per hour plus £150 per week expenses if the candidate lives outside of West Sussex

Location - Bridge House, Worthing

Job Description

The Social Worker will be responsible for assessments, reviews, safeguarding enquiries, and mental capacity act assessments. The successful candidate will work as part of a strong and dynamic social work service and will be required to undertake strengths-based assessments applying relevant legislation, utilizing social work skills and knowledge to develop and implement a plan of support aimed at improving wellbeing and delivering outcomes for people. The Social Worker will manage a caseload collaborating with people and family or friend carers, undertake strengths-based assessments and reviews, identifying eligible needs and personalized outcomes.

Key Requirements

  • Experience in working with frail, elderly people and older people with mental health needs
  • Experience of carrying out care act assessments and mental capacity assessments
  • A good understanding of the financial assessment process
  • Experience of strengths-based working
  • Good at following processes and managing conflict in terms of difficult conversations about management of resources
  • A driver/ability to travel around the county, including to care homes in rural locations
  • Able to work independently

Person Specification

  • Strive to remain up-to-date and compliant with all relevant legislation
  • Adhere to organizational procedures, policies, and codes of conduct as well as Social Work England professional standards to uphold a high-quality, seamless service
  • Work effectively in partnership with other staff and voluntary agencies

Qualifications and Experience

  • Qualified Social Worker with over 2 years experience
  • Registered with social work England
  • Must have experience of working with people needing/placed in residential and nursing care homes
