Social Worker
Lancashire County Council is seeking a highly motivated and experienced Social Worker to join their team as a Social Worker for Adults. The successful candidate will be responsible for conducting hospital discharge Triage/Duty, CHC MDT D2A reviews, and office-based assessments 2-3 times per week. The role is a lively position and requires an individual who is willing to work in a challenging and rewarding environment.
Key Responsibilities
- Conduct hospital discharge Triage/Duty, CHC MDT D2A reviews, and office-based assessments 2-3 times per week.
- Provide expert advice and support to service users, their families, and carers.
- Develop and maintain effective working relationships with colleagues, other professionals, and partner agencies.
- Ensure that service users receive high-quality care and support that is tailored to their individual needs.
- Participate in regular supervision, training, and development activities.
Requirements
- Qualified Social Worker with a valid SWE registration.
- Minimum of 2 years of experience as a Social Worker in an Adult setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work outside of normal hours when required.
