Adults Social Worker - Community Team - Stockport Council

Service Care Solutions are currently recruiting for a Social worker on behalf of Stockport Council's Community Team in the Adults services.

Working with partners across health, social care and the third sector you will play a vital role in supporting the people of Stockport.
Your main duties will be to provide an assessment, support planning and review service, underpinned by professional knowledge in order to ensure that:

• Vulnerable adults are safeguarded and are supported to live well with risk;
  * Person centred outcomes and support requirements are identified, and;
  * People are encouraged and enabled to live healthy and independent lives.

Pay rate is between £30 - £35 per hour dependent on level of experience.

Service Care Solutions are recruiting a Family Support worker on behalf of Cheshire East Council. The role is based in Crewe and will involve Supervision of Family Contact sessions.
