Job Title: Social Worker
Responsible for: Community Learning Disability Team
Duration: 5 Months
Hours per week: 35 Hours Per Week
Location: Elgin, Morayshire
Pay per hour: £31

We are looking to hire a Locum Social Worker to participate in the activities of an Integrated Community Learning Disability Team receiving requests for and under-taking holistic assessments of client's needs and commissioning packages of care working across professional boundaries when required. The start date would be ASAP and would look to be ending around the date of 31/03/24 but could be extended due to service requirements, down to the service leads discretion. Shift pattern would be 08:45am - 17:00pm Monday - Friday.

Day to day task and responsibilities -

• Co-ordinate the assessment of individual needs and prepare care plans to meet said needs
• Secure the resources or services required in responding to the identified needs
• Develop relationships with service providers/other professionals
• Co-ordinate the assessment of individual needs and prepare care plans to meet said needs
• Undertake Community Care assessments when required by Team Manager or on receipt of Referral from other professionals
• Design and organise suitable packages of care having undertaken financial assessments and ensure that adequate resources are available in conjunction with carers, relatives and statutory and independent sector agencies.
• Maintain case records, both electronic and manual, provide information for performance indicators, fulfil procedures and statutory duties in line with national and local policy practices and procedures.

What is required?

• Social Work Qualification - BA (Hons)Social Work or DipSW/CQSW or equivalent
• SSSC Social Work Scotland Registration

• Post Qualifying experience - 1 years and above
• Valid Clean Driving License
