We are seeking an experienced and motivated Band 6 Personal Health Budget (PHB) Practitioner to support our growing PHB service. This role offers the flexibility of remote working, with assessments and support planning primarily conducted virtually. However, there may be occasions where face-to-face visits are required, based on assessed need

Job Summary:
Key Responsibilities:
- Complete remote Care and Support Plans in line with PHB guidance and legal frameworks
- Facilitate and support individuals in developing person-centred support plans
- Ensure all plans are clinically appropriate, legally compliant, and financially sustainable
- Liaise with service users, families, providers, and multidisciplinary teams
- Promote the use of PHBs across Continuing Healthcare (CHC), mental health, and children’s services (as applicable)
- Contribute to the development and review of PHB pathways and policies
- Accurately record and maintain data in line with ICB requirements
- Demonstrable experience in completing Care and Support Plans
- Strong knowledge and practical application of PHB guidance and policy
- Excellent interpersonal and communication skills
- Ability to work independently and manage a complex caseload
- Comfortable using digital tools and conducting remote assessments
Additional Information:
- The role is primarily Monday to Friday, 9am–5pm, though some flexibility may be needed to facilitate person-centred planning and assessments.
- The ICB will minimise in-person contact where possible, supporting staff safety and efficient working.
- Equipment and remote access support will be provided.
