Location: Clowne (Chesterfield)
Client: Derbyshire County Council
Contract Type: Temporary (6 months)
Hours: Full time (37 hours a week)

About the Role
As a Community Care Worker, you will undertake routine visiting and assessment of clients under the direction of a Service Manager. You will support adults with less complex needs, helping them maximise independence, self-care, social and occupational opportunities, while ensuring safeguarding and access to appropriate services.

Key Responsibilities
  • Work with clients and their families/carers to identify strengths, networks, and opportunities to promote independence.
  • Collaborate with social workers, occupational therapists, and other professionals, referring on as appropriate.
  • Undertake holistic assessments and reviews of individual needs, creating care and support plans in line with legislation.
  • Arrange and commission appropriate services, including simple equipment and non-complex housing adaptations.
  • Disseminate information on available resources and link individuals to community-based activities.
  • Liaise with voluntary bodies, residential and day establishments, and other local agencies.
  • Maintain accurate records in line with Council policies.
  • Ensure the safeguarding of vulnerable adults.
  • Review work and caseload regularly with the Service Manager.
  • Uphold Equality, Diversity, Health & Safety, Risk Management, and Information Security policies.
Experience & Knowledge
Essential:
  • Direct experience of working with disabled and older people in a social care or healthcare setting.
  • Experience caring for or supporting others.
  • Experience working in a team, formally or informally, with other disciplines.
  • Experience devising and implementing care plans.
Desirable:
  • Knowledge of a range of aids to daily living.
  • Familiarity with health, education, welfare rights, and voluntary organisations.
  • Understanding of available local services and resources.
Skills & Competencies
Essential:
  • IT literacy with good written and verbal communication skills.
  • Strong organisational skills with ability to manage workload.
  • Ability to assess personal daily living needs and develop practical solutions.
  • Effective collaboration and negotiation skills with partner agencies.
  • Understanding of disability and the needs of older people.
  • Awareness of mental health impacts and the importance of positive risk-taking.
  • Commitment to confidentiality, independence, self-care, and empowerment.
  • Flexible and adaptable approach, aligned to Council policies and guidelines.
Desirable:
  • Counselling skills.
  • In-depth knowledge of welfare rights and community resources.
Qualifications
Essential:
  • NVQ Level 3 in Care (or willingness to work towards it if not already held).
Desirable:
  • NVQ Level 4 in Care.
  • General academic qualifications.
Other Requirements
  • Ability to speak fluent English to communicate effectively with clients and the public.
  • Ability to travel across service outlets throughout Derbyshire as required.
