About the Role
The Community Care Worker will carry out routine visits and assessments of clients under the direction of a Service Manager. The role supports individuals with less complex needs, helping them maintain independence and access appropriate services. The position involves working closely with families, carers, and other professionals to ensure effective care planning and delivery.

About the Role
The Community Care Worker will carry out routine visits and assessments of clients under the direction of a Service Manager. Key Responsibilities
- Work with clients and their families/carers to identify strengths and support networks.
- Collaborate with professionals such as social workers and occupational therapists.
- Conduct routine assessments and reviews of individual needs.
- Prescribe and deliver simple equipment and non-complex housing adaptations.
- Assist clients in creating viable care and support plans.
- Provide information on local resources and community activities.
- Liaise with voluntary bodies and other relevant agencies.
- Safeguard vulnerable adults.
- Maintain accurate records in line with DCC policy.
- Commission services in response to assessed needs.
Essential:
- Direct experience working with disabled and older people in a social care or healthcare setting.
- Experience caring for or supporting others.
- Experience working in a team with other disciplines.
- Experience devising and implementing care plans.
Essential:
- IT proficiency.
- Strong written and verbal communication.
- Practical aptitude and organisational skills.
- Ability to assess daily living needs.
- Understanding of disability and older people's needs.
- Awareness of confidentiality and safeguarding.
- Appreciation of mental health impacts and positive risk-taking.
- Flexibility and adaptability.
- Familiarity with Council policies and social care commitment.
- Knowledge of counselling, welfare rights, and available services.
Essential:
- NVQ Level III in Care (or willingness to obtain once in post).
- NVQ Level IV in Care.
- General academic qualifications.
- Fluent English communication skills.
- Ability to travel between service locations across Derbyshire.
